REPRESENTATION & FULL RELEASE FOR MINNESOTA GROWLERS WRESTLING CLUB
In consideration of the opportunity afforded me to use the services and facilities provided by Minnesota Growlers Wrestling Club in connection with my membership or contemplated membership in the Minnesota Growlers Wrestling Club, I freely and voluntarily make the following releases, statements and acknowledgments which I understand are legally binding upon me.

REPRESENTATIONS OF AGE AND PHYSICAL CONDITION
I declare and acknowledge that I am 18 years of age or older, and that I am in good health and capable of undergoing the physical exertion of wrestling and exercises for which I have voluntarily joined or contemplate joining. I have been advised by the club that if I have a medical history of any physical disorder or any doubts concerning the state of my health I should consult a physician before undertaking and exercise or wrestling for which I have joined the club.

ASSUMPTION OF RISK
I assume full responsibility and risk for any injury or damage which I might sustain directly or indirectly from any cause related to the wrestling program by the Club including, but not limited to, use of the various facilities offered by the Club. I hold harmless the owner(s) of the various homes, offices or facilities which the club has been allowed to use for its wrestling activities. I also acknowledge and agree that the Club or the owner(s) of the facilities being used are not responsible for personal belongings or holding or any loss or theft thereof, and I assume full responsibility and risk for any such loss or theft.

RELEASE FROM LIABILITY
I hereby release and forever discharge Minnesota Growlers Wrestling Club and each and all of the owners of any facilities used by the Club, their owners, heirs, assigns, agents, directors, officers, partners, employees, representatives, attorneys and all persons acting, by, through, under or in concert with them of and from any and all manner of action or actions, causes or causes or action at law or in equity, suits, debts, liens, contracts, agreements, promises, liabilities, claims, rights, obligation, demands, damages, losses, cost or expense, of any nature whatsoever, known or unknown, fixed or contingent (collectively referred to herein as “claims”), which I now have or may hereafter have against each or any of the above persons or entities by reason of any matter, cause or things whatsoever from the beginning of time to the date of this document, and specifically including any claims which I may now have or hereafter have against each or any of the above persons or entities by reason of my use of the facilities used by me as a member or contemplated member of Minnesota Growlers Wrestling Club. This release is binding upon me and upon my successors, heirs and assigns, and I intend this to be a complete release and discharge of all persons as well as any corporate entity having anything to do with my wrestling activities offered by Minnesota Growlers Wrestling Club or the operation and maintenance of the Club.

EFFECTIVENESS OF THIS DOCUMENT
This document is effective on and as of the date upon which I have signed it without regard to whether or not I joined the Minnesota Growlers Wrestling Club.

USE OF THIRD PARTY FACILITIES
As a tenant renting a public or private third party facility, Minnesota Growlers Wrestling Club and it's members shall abide by all facility guidelines as provided by the facility.

AGREEMENT
I HAVE CAREFULLY READ THIS ENTIRE DOCUMENT BEFORE I ELECTRONIC ACKNOWLEDGE THIS FORM. I FURTHER AGREE TO WET SIGN THE ORIGINAL PAPER DOCUMENT IN PERSON AT THE CLUB PRIOR TO GETTING ON THE MATS AND OFFICIALLY BECOMING A CLUB MEMBER. I FULLY REALIZE THAT THIS IS A BINDING LEGAL DOCUMENT WHICH AFFECTS ANY CLAIMS I MIGHT HAVE HAD RESULTING FROM MY USE OF THE FACILITIES OFFERED BY MINNESOTA GROWLERS WRESTLING CLUB BEFORE, DURING AND AFTER MY JOINING THE CLUB.
